*To enjoy the privilege of participation in extracurricular activities and competition at Peoria Public Schools, each individual must read, understand and adhere to the following rules, regulations, responsibilities, conditions and procedures of the Extracurricular Competition Code, The IHSA Sports Medicine Acknowledgement & Consent Form, and Athletico Physical Therapy Consent to Treat.
As part of the following registration process, the Extracurricular Competition Code will be signed by the individual athlete and parent/guardian. In addition, as part of the registration process, a current Pre-Participation Physical Examination Form signed and dated by a physician will be submitted electronically. Both the Extra Curricular Competition Code and the current physical examination must be on file with the school before he/she practices. The Pre-Participation Physical Examination is good for a period of 13 months from the date of the exam (a copy of the form may be downloaded from the "LINKS" section below).
Once you have the Pre-Participation Physical Exam completed by a physician, we encourage you to upload the completed Pre-Participation Physical Examination directly as part of completing registration with FAMILYID, otherwise a paper copy is acceptable but must be submitted to the coach of the sport you are registering. Keep in mind that paper submissions of the Physical will delay the completion of the registration process and may prevent the athlete from practicing immediately with the team.
Please register below, all fields must be completed in order to successfully complete registration, which includes the submission of a current physical. Failure to complete registration will result in the student/athlete not being able to participate in practice.
