All athletes that are participating in Fall sports must register online (no more handing out and collecting emergency information sheets). The only paper that coaches must receive is a Blue or Red Card, pertaining to their physicals. A parent/guardian should register by clicking on the green button below, and then follow these steps:
- Click on the link for the season's registration form. Scroll to bottom of page and click on Sign Up as a Family or Individual.
- Sign Up for your secure FamilyID account by entering your family name, email address and password. You will receive an email with a link to confirm your new account. (If you don't see the email, check your spam or junk folders)
- Click on the link in your email confirmation and then fill out the information requested on the registration form. (Payment will be handled separately from on-line registration)
- (Please be sure to update the grade you are entering for the school year.)
Physical exams must still be submitted to the Berlin High School Nurse's office prior to any participation. A physical exam must be current (within the past 13 months – 15 months for the Fall of 2020) and signed by the physician. Students will receive a Blue Card or a Red Card, which they will hand directly to the coach on the first day of practice. If a student athlete obtains a Blue Card from the nurse the student athlete has been medically cleared to participate in their sport for that season. If a student athlete obtains a Red Card from the nurse the student athlete will have a physical which expires during their season and will need to update their physical. If the physical examination expires during the course of the sports season, the athlete will be required to be examined prior to the date of the physical examination's expiration, otherwise continued participation is prohibited. Student athletes will be required to obtain a blue or red card prior to each season. If their physicals are good for the year, they must still go to the nurse’s office during the school year and receive blue or red card to hand to their winter or spring coach for that season.
Students can pick up their Blue or Red Card from the nurse and/or drop off their physicals to obtain a Blue or Red Card during the following designated days/times listed below. Please maintain social distancing and wear masks.
August 11th (FOOTBALL and Cross Country – ONLY) from 9:00-1:30 in the Front Lobby at BHS
August 18th (all other fall sports) from 9:00-1:30 in the Front Lobby at BHS
In the event that a card cannot
be picked up on the above dates/times, please email Leah Carbonell (email@example.com) to request one (a current physical must be on file for her to be able to do this). The Blue or Red Card will be left in the main office for you to pick up at a later time or will be given to your head coach.
You must use the State of Connecticut Health Assessment Record "HAR-3 Rev. 4/2011 for a physical. It is available in the BHS Nurse's office, your doctor's office, or on the BHS website. The physician must check the box on that form that states, "This student may participate fully in athletic activities and competitive sports." Please give this form directly to the Nurse's office, not to your coach.
