Acer 751T Purchase for Lost or Damaged Chromebook

Dear Parent/Guardian:

Your child has lost or damaged his/her Acer 751T district-issued Chromebook.  As noted on the Chromebook Use Agreement signed by all East Williston students and families, "All Chromebooks are covered for one breakage or one loss over the (4) years for which a student will be assigned a device.  Should more than one breakage occur over the four year period, or more than one loss occurs over the four year period, the student’s family will be billed to repair or replace the device."  Based on your child's loss or breakage, the Acer Chromebook needs to be replaced.  You can use this Family ID Form to remit payment. 

In order to pay for the lost or damaged Acer 751T Chromebook, you will be prompted to pay for the device using your My School Bucks account at the end of this Form. If you wish to pay by check, please contact Ed Kemnitzer at kemnitzere@ewsdmail.org.
