If you check "yes" to any of the 6 supplemental health history questions on the section 7, you will be required to fill out the general clearance form or the section 8.
Click HERE for a section 8.  This form will require a visit to a medical professional to be re-certified. If your child has sustained a concussion they must be cleared by an authorized medical examiner and by our Athletic Trainer following the WCSD concussion protocol and the return to play procedure.

Also, please make sure you are prepared for the first day of practice.  Information will be available at each of our schools.  You may not participate in a practice or game/scrimmage until all the required paperwork has been completed.  If you show up on the first official day of practice and forms are not all complete with necessary signatures, you will sit and watch practice until all requirements have been met.  If the sport you signed up for has try-outs you may not make the team.  PLEASE BE PREPARED.
