Welcome to the Peninsula High School Winter Sports 2019/2020 Registration. Students must complete this registration and pay the fees listed below. (Registration will close briefly from the 14th to the 18th to allow for clearing) Registration will reopen Monday morning 11/18, for non-cut sports (wrestling, boys swim, bowling).
All PHS Winter sports begin November 18th except Girls Bowling which will begin November 4th. Athletes must be cleared to practice on the 1st day.
What do you need to be cleared?
* ALL FEES AND FINES PAID- $50 ASB (Yearly) $100 Pay to Play (Per sport)
* PHYSICAL GOOD THROUGH STATE TOURNAMENT OF YOUR SPORT (Not negotiable) (Check state dates here: http://www.wiaa.com/)
* ANY MEDICAL FORMS FROM DOCTOR (Asthma/Epi-Pen etc.)
Sports with cuts possible will not be eligible for payment prior to cuts being made. These sports include Girls & Boys Basketball. Please plan to have Pay to Play and ASB fees paid by the first day of participation or have arrangements made with the bookkeeper for non-cut sports.
Eligibility:
To be eligible to participate in Sports and/or Activities at Peninsula:
-Student had a 2.0 GPA or above on the last grading period transcript and failed no more than one class. Failure to comply with this will result in a 5 week probation in all sports per WIAA rules.
-Student must pay all currently due FINES.
-Student must reside with parent(s) or legal guardians in this high school attendance area.
-Student must be enrolled in a school program and in regular attendance the first 15 days of the current term.
-Student MUST pass a physical exam during the last 24 months. A copy must be on file in the Athletic Office.
The physical on file cannot expire during the duration of the sport season. Parents can request a copy from the middle school or doctors office. The athletic office cannot do this due to HIPAA laws.
-Medical Insurance MUST be maintained throughout all sports participation. If you intend to purchase accident insurance through school insurance provider Myers, Stevens and Toohey, please write MST Insurance in the spaces below. Verification of policy activation will be required. (A copy of the acceptance email once enrolled, copy of provided cards etc.) These will need to be provided before participation is allowed.
PARENTS ARE RESPONSIBLE FOR ENSURING A GOOD PHYSICAL IS ON FILE. Check with your doctors office if you are in doubt TO CLARIFY: The Physical on file must be good through the championships of the sport season or your students registration will not be cleared until such time as a physical is provided that will be good through the championships. This is not open for debate.
** Medical conditions such as asthma and allergies that require an Epi-pen are the number one reason that registrations get held up. Please make sure you have all the proper paperwork on file with the nurse so that your student athlete can begin practice with everyone else. All Forms can be found on the Family ID home page.
Below, please answer all eligibility questions accurately. Falsifying information can result in a suspension from the sports program. Parents MUST sign off on all agreements/policies at the bottom of this registration. This cannot be completed by the student athlete even if the student is 18 or older. Failure to comply with this section will constitute falsification and can cost the athlete suspension for the entire season.
