PLEASE NOTE: Before any student can participate in tryouts for a sports team, the following requirements must be met:
PHYSICAL FORM(S): Student athletes must have a current physical on file in the Health Office in order to participate in Jonathan Dayton athletics. The New Jersey Department of Education Pre-participation Physical Evaluation Form must be used for the physical examination and is required once per year, to reflect a medical examination date within 365 days prior to the first practice session. Please note that Pre-participation Physical Evaluations done after May 15, 2015 must be completed by a health care provider who has completed the Student-Athlete Cardiac Assessment Professional Development Module.
The New Jersey Department of Education Health History Update Questionnaire must also be submitted for the 2nd and 3rd season of play (if you have already played a Fall sport), signed and dated 90 days prior to the first practice session.

PLEASE NOTE: THESE FORMS MUST BE SUBMITTED TO THE HEALTH OFFICES IN HARD COPY BY THE DEADLINE DATE REQUIRED FOR EACH SPORTS SEASON.
WINTER SPORTS PHYSICAL DATE: We will host free winter sports physicals on October 31st from 8-12. Student's interested in having a physical must submit the NJ Physical forms to Nurse Graham in the health office. Forms can be submitted no later than 10/29 in order to be eligible for a physical.
Parents/guardians are responsible for filling out all forms completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Health Services Department, Athletic Department and coaches.
Asthmatic / Inhaler users and Food Allergy Athletes:
All athletes who may be carrying an inhaler or an emergency epinephrine injection (for severe food allergies) must complete the required forms in order to self carry. The forms "Asthma action plan" and "Allergy action plan" can be found on the family ID page under "links." Please download these forms for your health care provider to complete. These forms should be completed once a year and submitted to the school nurse. The forms will be considered valid for one academic school year only. Please contact Mrs. Graham the Jonathan Dayton School Nurse should you have concerns or questions.
IMPACT TESTING: Student athletes must have a baseline ImPACT test on file with the JDHS database (baseline ImPACT tests expire after two years.) All incoming freshman and juniors are required to take an impact test. Please read the information posted in the links below about ImPACT testing for more information on Impact test administration.
