In order to be eligible to play sports at Wapato High School, you will need to complete this form, turn in a copy of a current physical to the office and pay fees to the office. When all of the requirements have been meet, you will be "eligible" to turn out.
Have passed a physical examination from a medical authority approved by law. The current physical must be signed and is good for two years (24 months) from the date the physical took place.
Medical Emergency/Parent Consent/Insurance Form
Medical emergency information, parent consent and updated insurance must be completed and signed during each registration period. Insurance coverage must included the minimum provision list on the parent consent form. I understand that my son/daughter cannot participate in after school athletics unless the School Accident Coverage Plan or your family plans covers him/her.
The parent/guardian must sign on behalf of herself/herself as well as the student-athlete (a minor) each registration period.
A "Risk Letter"
The parent/guardian must read the risk letter and sign its related agreement on behalf of himself/herself as well as the student-athlete (a minor) each registration period.
Sudden Cardiac Arrest and Concussion
Student-athlete and parental must read and sign the educational information in Concussion Management and Sudden Cardiac Arrest Awareness.
