WAITLIST - Rodger Quist Boy's Basketball Registration 2021/22

No student shall represent their school in interschool athletics until this registration form is on file with the Athletic Department as well as a signed physical form certifying that he/she has passed an adequate physical examination within the past year, noting that in the opinion of the examining physician, physician's assistant, nurse practitioner or a certified/registered chiropractor, is physically fit to participate in middle school athletics; that student has the consent of his/her parents or legal guardian to participate; and, the parent and participant have read, understand and agree to the CHSAA guidelines for eligibility.

An athletic fee will be assessed upon registration.   ($100 for SD27J students; see Fee Reduction for Free/Reduced Lunch; $195.00 for non SD27J students) It is required to be paid in full, before any athlete can participate in practice/tryouts, as well as all student fees paid in full per District 27J Policy. All fees can be paid online on the district website ($ Student fee payments tab). If paying with Check/Cash please bring payment into the Main Office.

**NOTE: There will be a 24 hour window before you are cleared to participate in a practice.**


FEE REDUCTION (FREE OR REDUCED LUNCH)

 STUDENT TRANSFERS

If you are a transfer student from another middle school, played sports for another middle school, or an exchange student you will be required to notify the athletic director and to fill out additional paperwork and turn into the main office. Please see CHSAA links to your right.

HOMESCHOOL/ONLINE/ATTENDING ANOTHER SCHOOL

When turning in your athletic paperwork, please include your last semester grades, contact information for attendance confirmation and proof of residency. (Note: your address of residency will determine the school you should attend for athletics.) Please see CHSAA Form 6 link to your right.

ELIGIBILITY FOR PARTICIPATION

Eligibility will be based on two different criteria; academic grades and RISE/ROAR.  On Tuesday of each week, academic eligibility will be pulled and determined by grades in all classes.  At this point, if a student has 2 Ds or 1 F, they will need to complete an eligibility rubric.  Eligibility will be re-evaluated on the next Tuesday.  Staff may show data that an athlete is not showing RISE/ROAR in their daily classrooms and this could cause an athlete to miss practice/contests, due to ineligibility.
