Welcome to the Van Wyck Junior High School Final Confirmation of Sports Participation,
This is the seasonal sports registration form that includes Health Update questions required for sports clearance. All athletes MUST submit this form to be cleared for sports.
Please read the following guidelines to understand the steps you must take to have your athlete approved to participate in sports in New York State.
All athletes must have:
- An approved physician's physical dated within the last 12 months
- An approved sports registration form containing updated health history information and athletic consent forms
New York State requires that any student in grade 7-12 who wants to try out for an interscholastic sport must first be cleared for participation by the WCSD physician or your private physician. The private physician physical examination must be submitted to the student’s school Health Office for review and eligibility determination by the District physician/associate.
In order to achieve the highest level of health and safety for all students, New York State Law also requires that the health office review a health history update of each athlete within 30 days of the start of the season.
To comply with New York State law and to provide our health offices with the time and resources necessary to clear athletes by the start of try-out's . We have two forms, the first, Sports Registration, provides a place for you to indicate your athletes interest in playing sports and allow the health office to review your athlete's physical several months before the start of the next season. The second, Final Confirmation form, will open exactly 30 days prior to the start of the season.
PLEASE NOTE: Your athlete will not be cleared to play without a valid physical on file and an approved Final Confirmation Form. Both forms should be submitted through FamilyID.
