UPDATED ATHLETIC PARTICIPATION ONLINE FORMS
Athletic Participation online forms must be completed, signed and uploaded to FamilyID prior to students trying out and participating in Sports.
Medical: Per CIF rules, an athlete must have a current physical on file (dated by physician after July 1, 2020). If you have had a physical during the 2019-2020 school year, your physician must sign a new medical release form.
Athletes with Kaiser Medical Insurance: Per CIF rules, a medical clearance is valid for one year from the date of the exam. Kaiser medical exams are given every two (2) years. This has caused confusion. Between exams, ask to have the Kaiser physician provide a current date next to his/her signature so the medical clearance will be acceptable. With the Kaiser medical clearances, the date of the physician’s signature will be used to determine the length of validity of the medical clearance.
