SUMMER PROGRAMS WILL NOT BE OFFERED UNTIL KING COUNTY REACHES PHASE 3 OF THE SAFE START WASHINGTON PLAN.
HOWEVER WE WANT TO ALLOW THOSE INTERESTED IN SUMMER PROGRAMS THE ABILITY TO SIGN UP AND BE READY IF/WHEN KING COUNTY MOVES TO PHASE 3.
IN ADDITION, NOT ALL SPORTS MAY HAVE A SUMMER PROGRAM OFFERED. PLEASE VERIFY THRU YOUR SCHOOL ATHLETIC OFFICE THAT THE SPORT YOU ARE INTERESTED IN IS AVAILABLE BEFORE PROCEEDING WITH REGISTRATION.
1. Complete this online registration
2. You must have a completed Athletic Pre-Participation History/Physical Examination form on file with the school Athletic Office that certifies that the participant’s physical condition is adequate for the activity or activities in which the student wishes to participate in. If you are an incoming 9th grader or a current student that didn't participate in sports during the 19-20 school year you will need to upload a current physical.
3. All athletes who wish to participate in summer athletics & activities must be covered either by a Student Accident Insurance Program or a separate insurance program. More details are included in the insurance area of this form.
4. Summer transfers from out of district are not eligible for summer activities until a transcript has been provided and the student is officially enrolled and approved by the Building Athletic Director. Due to limited staffing during the summer months it is possible that a transfer student will not be able to participate in summer activities.
Parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department & coaches.
