PLEASE REVIEW ALL INFORMATION BELOW THOROUGHLY TO AVOID ELIGIBILITY ISSUES AND RESTRICTIONS FROM PARTICIPATION - Thank you!

To enroll on a team, this FamilyID registration form must be completed, a hard-copy of the student's current physical must be submitted to the Athletic Office by Wednesday, March 11th.

This form represents a permission form and MUST BE COMPLETED BY A PARENT or GUARDIAN

Please be sure that all medical information- physical exam date, medical history, and especially head injury information- is ACCURATE and CURRENT

Spring 2020 Sports begin on Monday March 16th. The DEADLINE for FamilyID, and copy of Medical Physical is Wednesday, March 11th. 

Franklin High School is pleased to offer the following sports for the Spring 2020 season:

COPY OF CURRENT ANNUAL PHYSICAL
 The MIAA requires that all student-athletes have up to date/current physicals on file during an entire sports season. Any student-athlete whose physical expires in season will not be allowed to continue participating until a new physical is on file with the school nurse. Athletes without up-to-date physicals will not be allowed to participate until a current physical is turned in. 

COMPLETE ONLINE CONCUSSION COURSE
Parents and students-athletes, who plan to participate in any Franklin High School athletic program, must complete a free on-line concussion course. The entire course, including registration, can be completed in less than 30 minutes and can be found at: https://nfhslearn.com/courses/...

*Note: There is also an additional Impact test that must be taken in school at some point during the season. If you have already taken it for a Fall or Winter sport this year you are exempt. Dates and times to take the impact test will be determined once the season begins.

FEES

User Fees will be due the week of March 30th with the exception of Boys & Girls Outdoor Track. Fees may be paid by check or on-line via Unibank. Checks may be made out to The Town of Franklin may be turned in to the Athletic Office.


FINANCIAL ASSISTANCE
Financial assistance for the athletic fee is available for those that participate in the free/reduced lunch program. Please contact the Athletic Office if you are in need of assistance. 

CONFIRMATION
When registration has been completed, you will receive an email confirmation letter from FamilyID. Please note that paying the athletic fee on Unibank alone is not registering. Family Id and Unibank are separate. Family Id is our registration portal and Unibank is a payment portal. Remember to attach a receipt from Unibank to your child's physical.

 ***NEW*** STATE-MANDATED EDUCATION ON OPIOID MISUSE PREVENTION

New Massachusetts Opioid legislation requires that athletes and their parents/guardians receive educational materials on the dangers of opioid use and misuse.  Please review the following materials.  Electronic signature acknowledgment by both the athlete and parent is required in registration.

WHO NEEDS TO TAKE A BASELINE TEST?

a) ALL ATHLETES who took their last baseline for Spring 2015-16 or prior to that, or have never taken it.

b) Players who suffered a concussion since their last baseline test.  Post-injury ImPACT testing that was done is not the same as a new baseline.

Otherwise, baseline tests are good for 2 years.
