This program will allow for paperless registration and policy/form agreement. Your account will house your history and when you register for the next sport/activity, will only need to update changed information.
Before any student can participate in the athletic season, the following requirements must be met:
• All students MUST provide a copy of a physical exam dated within 12 months of the sport season start date. A health history update form, found on our website, must be completed and submitted to the high school nurse's office if the physical is more than 90 days old. Deadlines for this documentation for the spring season is March 1, 2020.
A Vision Screening must be accompanied with physicals.

Students with expired physicals will not be permitted to participate in tryouts or practices until a new physical is turned in.
• The parents/guardians are responsible for filling out the medical form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the nurse's office and athletic department.
• All agreements and policies listed here must be read and/or consented to to prior to the student athlete participating.
