Spring 2018 Mater Dei Prep Athletics Registration

Welcome to Spring 2018 Athletics Registration

Only a parent/guardian may complete the FamilyID® registration ensuring all medical information concerning their child is thorough and accurate. 

If  your child is prescribed  medication for asthma or food allergies (anaphylaxis) you will need to have the prescribing physician complete the "Physician Medication Order and Parent Consent" form to be turned in to the school nurse each school year.  Download medical forms below from "Links" .


You will need to print a copy of the "PreParticipation Physical Evaluation" (PPE) form from "Links" below, answer the questions on the HISTORY FORM and take ALL 4 PAGES to the licensed medical provider. 

A little background:

The Scholastic Student-Athlete Safety Act1 enacted in 2013 requires adherence to the following:

  • Student-athletes complete a medical/physical examination conducted not more than 365 days prior to the first day of the official practice in an athletic season using the "PreParticipation Physical Evaluation" (PPE) form.

  • The examining licensed medical provider be a physician, an advance practice nurse (APN) or a physician assistant (PA). The provider must have completed 1. The Student-Athletic Cardiac Assessment professional development module (PDM) prior to examining your child; and 2. During the examination/assessment the provider shall include a review of the “History Form” that has been signed by both parent/guardian and student. 

The PPE is a four (4) page form and Includes the following pages:

Page 1:  “History Form”. Parent/guardian and student complete this prior  to the examination.  Sign and date this page.  Insert the actual date of the physical examination at the top of the History Form. 

Page 2:  “The Athlete with Special Needs: Supplemental History Form”.  This is for a student with a disability that limits major life activities.  If this section does not apply, fill in your child's name and DOB and write N/A (Not Applicable).

Page 3 & 4:  “Physical Examination Form” and “Clearance Form”.  The health care provider who performs the physical examination must complete these two pages. The provider's signature, stamp and date are required on the form to validate the examination.  The health care provider who performs the physical examination must complete these two pages. Note:.  The health care provider's 2nd signature on the Clearance form is his/her certification statement attesting to the completion of the Cardiac Assessment Professional Development Module (must be signed and dated by the provider).

1Scholastic Student-Athlete Safety Act was enacted in June of 2013 concerning the health of student-athletes, amending P.L. 2007, c.125, and supplementing Title 18A of the New Jersey Statutes.  


New from NJ Department of Education:  In July 2017, P.L. 2017, c.167 was signed  into law requiring the development of a fact sheet containing information on the use and misuse of opioid drugs in the event that a health care provider prescribes an opioid for a sports-related injury. 

In accordance with N.J.S.A. 18A:40-41.10, Mater Dei Prep is distributing the fact sheet entitled, "Opioid Use and Misuse Educational Fact Sheet" to all student-athletes and cheerleaders prior to the first official practice of the school year.  Therefore, the fact sheet and sign off sheet have been incorporated into the FamilyID on-line registration form as an Agreement and both the student athlete/cheerleader and the parent/guardian review and sign off acknowledging receipt of the fact sheet.

{ REMINDER:   The valid PPE is no longer uploaded into PARTICIPANT section, they must be given to the school nurse once completed.}

1.  Provide to the school nurse the completed valid PreParticipation Physical Evaluation (PPE) exam.  It is valid for one year from the date it is completed and must be repeated on an annual basis for sports participation.
