IMPORTANT UPDATE for Spring Sports/Tryouts:
The MIAA has informed member schools that the start date for Spring Sports has been pushed back to Monday, May 4th.  We will continue to monitor the situation, but tryouts/practices will not begin until that date.  More information to follow as it becomes available.



Dear Athletes and Families,

We are excited to announce that we are offering the convenience of online registration for our sports programs through partner company, FamilyID ( FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible.

Parents/guardians register through FamilyID, entering accurate and up-to-date health history for each sport in which a student athlete participates. This provides the school athletic trainer with the most current information regarding concussions, injuries, or circumstances that may affect a student athlete since their last physical examination and/or sports season. Enrollment in FamilyID indicates parental consent for participation.


1. All student athletes intending to participate in school sports are required to receive an athletic physical with their own physician. SBRHS does not offer free physicals before each sports season. The physical exam will cover the student for 13 months from the exam date. A student's eligibility will terminate once a physical has reached the 13 month limit, no exception. (MIAA Handbook, part 56.1)

2. Students may not participate in tryouts, practices, or games without a medical clearance from the athletic trainer;
which includes

A. Registration on FamilyId (You will get a email confirmation with the sport you signed up for)

B. Completing Impact Testing (You can sign up from this link )

C. Having an up to date physical on file with the athletic trainer 

  • need a signature from either a Licensed Physician, Physician’s Assistant, or a Nurse Practitioner
  • Needs to state ‘athlete is cleared for sports participation’
  • All physicals need to have weight, blood pressure, and height documented
  • Date of physical must be present
  • Physicals are accepted for 13 months from the physical date- extensions are NOT given for any reason.
  • All physical exams need to be an actual physical, NOT a sick visit that has been amended. This is a district policy and a MIAA Mandate
  • The athletic trainer may not be able to get physicals from the school nurse at times, please make sure a copy is handed in to her office.

Students can look for their clearance status on the office door of the athletic trainer  nurse or athletic director.

All of this must be completed and/or passed into the main office at 625 County St. Somerset, MA 02726 - Attention Susan Medeiros - by March 2, 2020.   Paperwork received after March 2nd will need minimum 24 hours to be processed and clearance not guaranteed for pre-season start dates. The school nurse and athletic trainer reserve the right to require a specific medical clearance from a specialist (orthopedist, neurologist, cardiologist, etc.) based on the updated health history. (You would be contacted via the email address given on FamilyID.)

3. For legal reasons, official paperwork from a physician's office may not be altered or falsified by a student and/or a parent/guardian. This includes but is not limited to dates, signatures or content. Further, students are not allowed to sign or forge where a parent/guardian signature is required. Falsifying a student's medical record is criminal in nature and will not be tolerated. Infractions will affect the student's participation in his/her sport.

*******NOTE: Faxed copies of physical exams or medical clearances must be faxed directly from the examining physician's office, not by a parent/guardian, in compliance with MA DPH guidelines. Please fax to the Main Office at 508-324-3118 Attention: Susan Medeiros.


User fees must be paid, unless fees have been waived for students who receive free or reduced lunch using the Massachusetts eligibility standards established for free and reduced price meals or a financial hardship waiver for user fees has been approved.

The Somerset Berkley Regional School District School Committee has authorized the establishment of an athletic and extracurricular fees program as a supplemental revenue source contributing to a percentage of the School District’s total costs for operating school-sponsored high school athletics and activities. The fees collected are used to help pay stipends for coaches and advisors, to defray the costs of purchasing and/or maintaining equipment, supplies, uniforms, transportation and the costs associated with proper maintenance of playing fields, courts, or practice areas.


The payment of the Athletic Participation Fee to the school carries with it no expectation of playing time for the student-athlete. Coaches are solely responsible for the decisions of playing time based the development of the student-athlete’s skills, their adherence to school, team, and Somerset Berkley Regional High School rules, and their exhibition of proper sportsmanship.

For extracurricular activities, the user fees structure is as follows:

1ST SPORT $150.00

2ND SPORT $100.00

3RD SPORT $0.00  

Parents, students and coaches should be aware that the athletic fees do not cover equipment that is typically purchased personally by athletes or parents, including but not limited to footwear, practice clothing, and specialized equipment preferred by the student-athlete.

Payment Terms

Athletic fees will be processed through the FamilyID season registration. All payments must be made online through the site. Cash and checks will not be accepted for payment.  For those students who are not selected for the final team roster, they will be issued a refund electronically in the event they have already paid their fee.  If the athletic and extracurricular fee is not paid in full or waived pursuant to an individual free and reduced application or a financial hardship waiver, then the student-athlete cannot play on the team or participate in the activity.  Payment is due before the first regular season contest.


Fees are automatically waived for students who receive free or reduced lunch using the Massachusetts eligibility standards established for free and reduced price meals. The student/family may receive a letter by the end of August indicating their student will receive free lunch for the upcoming school year. If a letter is not received by student/family, the student’s parent or guardian is encouraged to complete the Application for Free and Reduced Breakfast and Lunch sent home to every student at the beginning of the school year. Once this application is processed through the Office of the Superintendent, a notification will be sent home for each family indicating if free or reduced lunch is available for their student(s).

 If the athletic or extracurricular user fee is considered a financial hardship for the student/family, an athletic user fee waiver form may be completed for any student/family who does not qualify for free and reduced lunch. If a student/family feels they cannot afford the athletic or extracurricular fee, they may request a payment plan, an extension of the due date, or a waiver.

All requests should be submitted to the Principal using the Athletic and Extracurricular User Fee Waiver Form located on the high school’s website.  Strict confidentiality will be maintained.


The athletic and extracurricular user fee is non-refundable.  For athletics and clubs, any participant who leaves a team or club voluntarily or who is dropped from a team or club by the Coach, Athletic Director, Advisor or Principal for disciplinary of scholastic deficiencies is not eligible for a refund. The Principal shall be authorized to approve an athletic and extracurricular registration fee refund to a pupil for a reason(s) not listed above in extenuating circumstances.


The state law requires that student/athletes along with their parent(s)/guardian(s) complete the concussion course. During registration you will be asked to sign off that you have completed one of these courses.  This is different from the ImPACT test.  Prior to their participation in each sports season, Parents/Guardians shall affirm on FamilyID, that they and their son/daughter have completed either the NFHS of CDC online concussion course listed below:
