Welcome to RHAM Middle School Athletics Registration. We are excited to offer the ease of online registration.

Completion and submission of this form must be received before the student-athlete may participate in any school team-organized practice , scrimmage, or regularly scheduled game contest.

Physical Exam/Health Assessment Record

Please note it is mandatory that all students have a valid physical exam/health assessment form on file with the Athletic Director's and Nurse's Office prior to athletic participation (Physical exams expire after 13 months for athletic participation). Physical exam/health assessment forms can be submitted via this registration portal by uploading a scanned image or photo of the required forms in the "Participant Information Section-Physicians". The forms may also be submitted to the Athletic Director's office, where it will be processed and turned into Nurse's office (the form should go to Athletic Director's office first if being used for athletic participation).

Page 1 of form must be filled out completely and signed by parent. Page 2 must be completed and signed by student's medical provider.

Participation Fees

By action of the Region 8 School District Board of Education, a participation fee of $170.00 will be charged to all participating student-athletes per athletic season. The annual family cap is $510.00. After a family incurs $510.00 in participation fees, all future athletic participation fees for the school year will be waived. Fees are required to be paid prior to the first competition. In sports where try-outs are held, payments will not be accepted until the coach of that sport finalizes their roster.

Students who are on free or reduced lunch plans with district will pay the following athletic participation fees:

Reduced Lunch: $60.00

Free Lunch: $40.00
