Welcome to Registration for the FALL 2018-2019 Athletic Programs
Note: Please use GOOGLE CHROME browser, if you don't you may have technical issues/glitches completing the registration.
TO PARTICIPATE for the First Day of FALL Practice, You Must Complete the Following:
- A Physical, PERFORMED on or after JUNE 1, 2018 must be ON FILE with Jerry Dancho, HS Athletic Trainer.
- COMPLETE the FAMILYID Rregistration.
Parent/Guardian, please complete the registration form below. Once the student-athlete has been cleared by the Athletic Department and Athletic Trainer, their name will be listed on the clear to participate list. Please turn in a hard copy of Section 5 & 6 Physical form to the middle school Athletic Trainer, Athletes must comply with a 24 hour waiting period (to process medical forms) after the receipt of physicals prior to being permitted to practice.
PAY 2 PLAY FEE – The District's Pay-2-Play Fee must be paid prior to the completion of the second week of the season. The Pay 2 Play Fee is to be paid online during the Payment Period:
- To pay on-line, use the "PAY ONLINE" tab at the end of this registration process. A one-time payment using a Visa, MasterCard, American Express or Discovery card is the easiest and quickest way.
- If you are EXEMPT (3 Fee Cap/Free or Reduced Lunch) or would prefer not to pay on-line, please complete the Student Activity Fee Checklist Form for the activity of participation with all information requested, attach cash, check or supporting waiver documentation, if required, and submit to the Athletic Director, Sylvia Kalazs, at the High School. Please write your CHILD’S NAME and ACTIVITY in the memo portion of the check. Checks are made payable to Quakertown Community School District.
