This is the online registration for Penn-Trafford High School's Student Emergency Information. In the past, students have been given Emergency Cards to take home and have their parents complete and then return. This new FamilyID registration process will take the place of the Emergency Cards.
ALL PTHS Student Families MUST complete this Emergency Information Registration, just like the paper Emergency Cards.
The FamilyID online registration will allow you to make changes to your student's Emergency Information at any time during the school year. Notification will be sent to the appropriate school personnel to update your information in PowerSchool. You will not have to make additional notifications.
Please be sure to enter all the required information as you will not be able to submit your student's registration until completed. If you have difficulty with your registration, please feel free to contact FamilyID Support at email@example.com or 888.800.5583.
Once your student's information is reviewed, if there is need for follow-up for and Individualized Health Plan due to a medical condition and you have not already submitted the appropriate forms and medical documentations, Nurse Lisa Popovich will contact you. If you have medical concerns for your student, please feel free to contact Nurse Popovich via email at firstname.lastname@example.org or 724-744-0302.
It is our hope that this type of online registration will make it easier for families to communicate necessary student contact and medical information with high school personnel. Please be sure to complete your registration by Monday, September 3rd.
