Patton Middle School 2019-20 - Spring Sports Initial Registration

This registration is for athletes who have not yet completed a FamilyID registration for the Academic year 2019-20. If your child has already tried out for or participated in a sport this school year, please complete the Patton Subsequent Season Registration. It is much shorter and quicker to complete.

Due date for all Spring sports: 2/24/2020

This registration must be completed and approved prior to any participation in tryouts, practices, scrimmages or games. The following information applies to the physical examination required for sport's participation:

INITIAL EVALUATION: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is required to complete a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE); the equivalent of Sections 1-5 of the CIPPE are completed electronically via your FamilyID registration. Section 6 must be completed by an Authorized Medical Examiner (AME). Once this section is completed and signed, it must be uploaded into your FamilyID registration in the Physician's Section. The CIPPE may not be authorized earlier than June 1st and shall be effective, regardless of when performed during a school year, until the latter of the next May 31st or the conclusion of the spring sports season.

PLEASE NOTE: FOR THE FIRST SPORT IN A SCHOOL YEAR, A COMPLETED SECTION 6 MUST BE UPLOADED BELOW UNDER THE "PHYSICIANS" HEADER. FOR ALL SUBSEQUENT SEASONS THE SECTION 6 DOES NOT NEED TO BE RE-UPLOADED, BUT YOU MUST VERIFY THE INFORMATION IS CORRECT AND AGREE TO THE PIAA AGREEMENTS BELOW. 

PLEASE NOTE: IF A SECTION 8 IS REQUIRED, IT MUST BE TURNED IN TO THE ATHLETIC TRAINER OR PATTON MAIN OFFICE.
