Welcome to Paramus High School 2017-2018 Spring Athletic/Student Activity Registration
All students who will participate in the athletic program, cheerleading, marching band, dance team or Unified Sports are required to obtain a physical examination prior to the first practice or tryout session.
All students participating in ski club, weight training or open gym are required to obtain a physical examination prior to the first day of participation.
Be advised that ALL physical examinations forms submitted must be reviewed by the school physician prior to participation. This process may take up to two weeks. Please plan accordingly when handing in paperwork and adhere to the announced due dates.
WE MUST HAVE THE ORIGINAL FORM WITH THE PHYSICIAN'S SIGNATURE.
SIGNATURE STAMPS ARE NOT ACCEPTED. YOUR PHYSICIAN MUST SIGN THE FORM.
ALL PHYSICALS MUST BE HANDED INTO THE NURSES OFFICE
It is recommended that you keep a copy of any forms signed by your physician for your own records.

-STUDENTS WITH DIABETES MUST SUBMIT ORDERS FOR DIABETES MANAGEMENT IN SCHOOL AND SPORTS FROM THEIR HEALTHCARE PROVIDER.
As per the Scholastic Student-Athletic Safety Act (P.L. 2013, c.71), the preparticipation physical examination (page 3) must be completed by a health care provider who has completed the Student-Athlete Cardiac Assessment Module. Please check with your health care provider before your exam to make sure they have done this. Physical Exams done by providers who have not completed this module will not be accepted and will be returned to the student.
