North Haven High School UNIFIED Sports Registration Packet 2017-2018

Unified Sports is a registered program of Special Olympics. It is a program that combines approximately equal numbers of athletes with and without an intellectual disability on sports teams for training and competition. All Unified Sports players, both athletes and Unified partners, are of similar age and matched by sport skill ability. Unified Sports teams are placed in competitive divisions based on their skill abilities, ranging from training divisions (focused on skill-learning) to high-level competition.

FORMS IN THIS PACKET CONSIST OF:

1.   PHYSICAL EXAMINATION:

Form must be completed before tryouts of a sport. This will remain valid for 13 months to date. This physical examination will cover ALL sports played during those 13 months.

2.   MEDICAL HISTORY:

Medical History Form must be completed and signed by the Parent/Guardian and Student.

3.   PERMISSION SLIP:

Must be signed by the Parent/Guardian and emergency information must be completed.

4.   CODE OF CONDUCT:

Must be signed each school year by the Parent/Guardian and the Student.

5.   ACADEMIC REQUIREMENTS FOR STUDENT ATHLETES:

Must be signed each school year by the Parent/Guardian and the Student.

6.   ATTENDANCE:

Must be signed each school year by the Parent/Guardian and the Student.

7.   CONCUSSION CONSENT FORM

Must be signed each school year by the Parent/Guardian and the Student.

8.   CARDIAC ARREST CONSENT FORM

Must be signed each school year by the Parent/Guardian and the Student.

9.   TRANSPORTATION:

Must be read and understood by the parent/guardian.

When you submit this completed registration form, it  will be sent directly to the North Haven High School Nurse. The school nurse will review all parts of this completed registration form, including the uploaded physical, the medical history, the permission slip, the code of conduct, academic requirements, attendance, concussion consent, and cardiac arrest consent. 

A new athletics registration form is required for every school year. In order to be eligible for the first day of tryouts, the completed registration form must be submitted online to the school nurse 48 hours before the start of the season (before the first day of tryouts). Forms not submitted within the specified timeframe may result in the prospective student-athlete not being eligible for the start of tryouts. *Please note, anyone submitting a registration form the same day tryouts/practice begin will not become eligible until the following weekday. A final roster of eligible players will be provided to the Athletic Director and coach the day before the first tryout/practice. The coach will also be notified of any ineligible player. The following dates are deadlines for each sports season:


INJURIES:

1.    The COACH and INJURED PLAYER must report all injuries to the NURSE the following day.

2.     Insurance forms and accident forms are to be completed in duplicate. The school nurse has the forms.

3.    WRITTEN RELEASE: If the athlete is seen by a health care provider, a written release is required before participation may be resumed.

4.   THE SCHOOL NURSE SHOULD BE INFORMED OF ANY CHANGE IN MEDICAL STATUS OF A POTENTIAL ATHLETE THROUGHOUT THE SCHOOL YEAR.
