Welcome Prospective Athletes and Parents,
For any of students to be eligible to tryout, practice, or compete for one of our athletic teams, we must have the following items completed and on file in our athletic department:
- Register for the prospective sport through Family ID. All athletes and families must complete the registration form and sign the athletic contract page stating they have read, understand, and agree to the terms of our athletic policies in our contract. This is done on the Family ID site. We no longer require papers to be turned into the Athletic Dept.
- Physical Exam on file with the school nurse. Upon entering 6th and 9th grade, all athletes must have proof of a current physical exam on file with our school nurse. For the 2020-21 school year, the physical must be dated after August 1, 2019 for incoming 6th and 9th graders. For new students playing sports, the date must be equivalent to the year entering 6th or 9th grade. You may email a copy to Athletic Director Jamie Hayes email@example.com or you can submit the form with your Family ID registration.
Complete Baseline Testing. Per school board policy, all athletes must complete baseline testing before participating in any contact sport. The test must be completed every two years. The Athletic Trainers at Access Sports Medicine will now administer all Baseline Testing at the start of the Athletic Season.
