PRE-PARTICIPATION PHYSICAL EVALUATION FORM & HEALTH HISTORY UPDATE
In order to participate in interscholastic sports at the middle school, 7th and 8th grade students must submit, or have in their health folder, 2 different medical forms. These forms can be found in the "Links" section of this online form where you can download them. These forms are to be turned in to the nurses' office by a predetermined date which will be announced as each season approaches.
There are no academic requirements for 7th and 8th grade students to participate. Please become familiar with academic requirements for the high school which are listed below for future reference.
- To be eligible to participate in sports under the NJSIAA eligibility guidelines during the Fall and Winter seasons, a student must earn 30 credits at the conclusion of the second semester of the previous school year.
- To be eligible for the Spring season, a student must have earned a minimum of 15 credits, or 50% of the required 30 credits, at the conclusion of the first semester. If any student is eligible at the start of a sports' season, he/she remains eligible for that season regardless of his/her grades at the end of a marking period.
- *All incoming freshmen are automatically academically eligible to participate in sports for the Fall and Winter seasons.
All athletes selected for teams after tryouts must complete baseline ImPACT testing before being allowed to participate in their sport.
ImPACT is a computerized concussion evaluation system that measures verbal and visual memory, processing speed and reaction time. To most effectively care for athletes who have sustained concussions, it is helpful to compare baseline date to post-concussion data so that any deficits can be determined and proper return-to-play decisions can be made.
