Middle School 19-20 Spring Athletic Registration

Welcome Athletes & Parents,

The PIAA Sports Physical forms packet (Comprehensive Initial Pre-Participation Physical Evaluation or CIPPE) has been updated for the current school year. To be eligible to play a sport during the current school year, the initial CIPPE physical evaluation must be completed no earlier than June 1 of the current school year.  If a physical was given prior to June 1, of the current school year it is not acceptable.

Only one comprehensive examination is required per academic year. This comprehensive evaluation can be performed at your physician's office. Sections 1, 2, 3, and 4 of the physical packet must be filled out by a parent or guardian and completed on-line. Section 5 is to be completed by parent and returned with Section 6 (completed and signed by a physician) to the MIddle School Office.

For any athlete participating in multiple sports within a school year, the Recertification Form (Section 7) needs to be turned in for the second and/or third sport played within 6 weeks of the start of that season. The Recertification Form simply updates any insurance or contact information and indicates whether or not the student-athlete has sustained an injury since his/her last physical. If the athlete has sustained a serious injury since his/her initial physical, then Sections 7 and 8 are required before the start of any subsequent sport.

It is recommended to have the physical evaluation done sometime after June 1 of the current school year, and before the first day of the high school fall sport practice, regardless of whether or not you plan on participating in a fall sport.

The following forms must be completed on-line: School District Forms: Code of Conduct / Sportsmanship (Form A), Waiver, Release, and Informed Consent (Form B), Awareness Form (Form C), Concussion Management Awareness Form; Allegheny Health Network HIPPA/Consent to Treat Form; PIAA Forms - Comprehensive Initial Pre-Participation Physical Evaluation: Section 1, Section 2, Section 3, Section 4.

The Peters Township School District contracts for Athletic Training Services through Allegheny Health Network. The Allegheny Health Network requires a HIPPA and Consent to Treat Form to be completed prior to participation.

Section 3 (Understand of Risk of Concussion and Traumatic Brain Injury) of the CIPPE contains information about concussions. Please take note of all information included. Additional information about concussions can be found at

Additionally, the Peters Township Athletic Department mandates that all students participating in athletics complete the Concussion Baseline Testing Program prior to the first day of official practice as outlined by the PIAA. This test needs to be taken every two years. Information about baseline testing is available through the Athletic Trainer.


In addition to what was previously stated, Section 9 must also be completed by our sports medicine department (Athletic Trainer) who will administer the initial assessment for wrestlers; and the school district will provide certification at a designated time by the school physician.


Please return the athletic fee with the form attached. High School fees should be sent to the Athletic Office. Middle School fees should be sent to the Middle School Office.
