Mexico Academy and Central School District Registration (Pre-K and Gr 1-12)

Student Registering Letter
Welcome to the Mexico Academy & Central School District.  We have five schools located within our District –Mexico High School (Grade 9-12); Mexico Middle School (Grade 5-8); Mexico Elementary (Grade Pre-K -  4); New Haven Elementary (Grade Pre-K – 4); and Palermo Elementary (Grade Pre-K – 4).

Registration may be completed in many ways; you may complete the electronic process below, download forms from the Resource section of the District Home page at www.mexicocsd.org, or picked up from any school, or from the District Office.

Please review the list below for the necessary items to complete the registration process.  Please note there may be additional information requested from your child's school.  All registrations are completed through the District Office.

You may either upload copies of the following documents or bring originals to be copied:

  1.   Birth certificate
  2.   Immunization Record (including lead levels for Pre-K)
  3.   Physical –All new/returning students
  4.   3- Proofs of Residency (Please see accompanied letter for appropriate forms)
  5.   Custody Papers (If applicable)
  6.   Proof of Guardianship (Court Ordered)
  7.   Child’s IEP (If child is receiving Special Education Services)
  8.   Child’s 504 Plan (If child is receiving 504 Accommodations)
  9.   Middle School/High School Students – copy of previous schedule and report card
  10.   Foster Students (social worker name/number and paperwork from County DSS included with registration packet.

At time of student registration, a student photo will be taken.

Kindergarten Registration Letter
Mexico Academy and Central Schools is beginning Kindergarten registration for the 2018-2019 school year, for children who are 5 years old by December 1, 2018. A student currently enrolled in the district’s UPK program is already registered and no additional paperwork is needed.  Registration packets can be picked up at any elementary building or the district office, on-line at www.mexicocsd.org, or by calling Terri Herrington, District Registrar, at 963-8400, ext. 5400. Registration packets are due to the District Office by April 6, 2018. Following that, you will be contacted to set up your Kindergarten screening appointment, which will take place in May. District Office Kindergarten Welcome
The Mexico Academy & Central School District is pleased to welcome your child to our Kindergarten program.  We have Kindergarten classes at New Haven Elementary, Mexico Elementary, and Palermo Elementary.  Enrollment is based on geographical location of your residential address.  Students who are eligible to attend Kindergarten must be five years old by December 1st, 2018. 

The Kindergarten program runs full day and five days a week.  Completed applications must be submitted to the District Office by April 6th.  Each elementary school will be calling during late April to secure an appointment for your student’s screening.  Screenings for Kindergarten will be completed during the month of May.  Therefore, you are encouraged to submit your registration as early as possible.  Verification of the application information may be required at a later date.

Enclosed you will find registration, immunization, and health forms to be completed for your child to attend Kindergarten.  The required immunizations for acceptance into the Kindergarten program are as follows:  4-5 doses of DTap/DTP/Tdap, 3-4 does of Polio, 2 doses of MMR, 3 doses of Hep B, and 2 doses of Varicella.  If your child is missing any of these immunizations in the required number of does, he/she will not be accepted into the school.  *Your child can not be registered without a completed immunization form.  A dental form is enclosed to be completed by your child’s dental office.  The completed application, along with all the required documentations should be sent to Terri Herrington, Central Registrar.  Applications may also be returned to your local elementary school and they will forward the forms to Mrs. Herrington.

Please note:  Due to NYS regulations, we must receive a report of a physical examination; completed within the last year, signed by a licensed health provider. The state also required that we receive proof of a blood lead level test.
