Back to School Health Forms 2019-2020 School Year
Class of 2020/Incoming Seniors:
Must have: the 2nd dose of meningococcal vaccine, (MenACWY/MCV4), in order to start school. Proof from your physician is required for attendance.
Class of 2021/Incoming Juniors:
Must have: Updated health examination and dental exam. These exams must be signed by your pediatrician and dentist and dated after 9/1/18. Anything dated before 9/1/18 will be considered expired and will require a state-mandated update by your physician.
Class of 2022/Incoming Sophomores:
There are no state requirements for 10th graders, however, all medications and athletic sign-ups will require either an updated medication form or an updated health examination form.
Please submit all forms no later than the first day of school in September.
- All students that would like access to over the counter medications such as Tylenol, Motrin, Advil, Benadryl or Tums, that are to be given out during the school day require a new Authorization for Administration of Medication in School Form to be filled out by a parent/guardian and signed by a physician for the 2019-20 school year. You may upload this completed form and submit in FamilyID or bring the hard-copy to the nurse's office.
- All students who self-carry their own EpiPens and inhalers must have a new Self Medication Authorization Form that is filled out by a parent/guardian and signed by a physician for the 2019-20 school year.
