1) Parents: must complete this form no sooner than 30 days prior to the first day of practice.
2) Attend the pre-season sports meeting for important information.
3) To be eligible for the first day of practices each athlete must have completed online registration and confirmed a current physical is on file with the nurses office.
A student who participates in interscholastic athletic competition must have a current health examination, complete this form, and obtain final approval school nurse. Athletes who register for sports and whom DO NOT have a current physical will be contacted by the nurses office. If you do not receive word, then your athlete is cleared to play. If you DO NOT have a current physical on file and you do not respond to action from the school, your athlete will not be allowed to play.
Physical exams are best performed by your own doctor, who has a complete history of your child and who will perform a comprehensive examination. Physical examinations will not be given by our district staff over the summer vacation (you must use your private physician).
The results of the physical shall be valid for a period of 12 months up to the last day of the month in which the physical was conducted. If the 12 month period for the physical exam expires during a sport season, participants may complete the season as long as a health history was conducted prior to the season. For example, a physical conducted on August 1 would be valid through August 31st. If a student plays beyond August (ex. football), the student may complete that sports season as long as an interval health history was conducted before the start of the season. Sports season includes tryouts.
