JMS 2017/18 Athletics Registration

The following items are needed for an athlete to have clearance for participation.


*Complete FamilyID Registration Form.

*Physical (physical is good for 2 yrs from time taken) must be brought to athletic office.

*ASB Card (must be purchased before the first try-out/practice of the season).

*Scholastic Eligibility (must be enrolled and passing 5 classes with a 2.0 GPA).

*Participation Fee (must be paid before the first try-out/practice of the season).

*All fines and fees must be paid before the first try-out/practice of the season.
