The Jamestown School Department (JSD) recognizes that its students need to be engaged in activities that promote 21st Century learning skills. JSD also recognizes that some of our students do not have access to technology tools beyond the school day, and that access to these learning tools facilitates extended learning beyond the traditional school day. JSD students may use district technology devices after they and their parents/guardians have signed the Technology Device Parent/Student User Agreement. Students wishing to participate must follow the policies stated in the JSD Acceptable Use Policy (AUP).
Please complete the following form for each of your Jamestown School Department students. This form requires you to read/watch and sign the following:

Jamestown Schools provides access to purchase optional insurance for District-loaned devices through a third party provider (insurance information). Some items may be covered by an existing homeowners/renters policy. Please check with your insurance agent about this possibility.
If you would like to purchase insurance, please submit this form first, then follow the instructions/links in your registration receipt. The enrollment window for insurance is 8/28/18 – 10/31/18.
