Welcome to online sign up for High School Students Intent to Participate for North Tonawanda Athletics.
Please complete the High School Athletics Intent to Participate Form if your child is planning to tryout for any school team for the 2018-2019 season.
The school nurse will review whether the student is in need of an updated physical, and will send you a link to your email provided to FamilyID to set up an appointment for a school offered physical.
Student-athletes must have a current health physical (that is valid for the entire upcoming season) on file prior to the start of the season. Physicals are good for a period of 12 months and expire the last day of the month in which the physical was conducted by the school district provider.
PLEASE NOTE: The Intent to Participate Form IS NOT the final registration form for participation in interscholastic athletics. This is the first step in the registration process. New York State law requires that all school districts review a student's health profile within 30 days of the start of the season, therefore the final step to register for participation in interscholastic athletics will simply require you to log back into your FamilyID account no more than 30 days prior to the beginning of the upcoming season and provide additional required information via Fall Sports Registration. Those who have completed the Intent to Participate Form will not be notified once the final registration period opens.
