HIGH SCHOOL - Strength & Conditioning 2020-2021

FORMS IN THIS PACKET CONSIST OF:

1.   PHYSICAL EXAMINATION:

Form must be completed before participating in any sport. This physical will remain valid for 13 months from the date of the physical examination. This physical examination will cover ALL sports played during those 13 months.

2.   MEDICAL HISTORY:

Medical History Form must be completed and signed by the Parent/Guardian and Student.

3.   PERMISSION SLIP:

Must be read and signed by the Parent/Guardian and Student. Emergency information must be completed.

4.   ACADEMIC REQUIREMENTS FOR STUDENT ATHLETES:

Must be read and signed by the Parent/Guardian and the Student.

5.   ATTENDANCE:

Must be read and signed by the Parent/Guardian and the Student.

6.   CODE OF CONDUCT:

Must be read and signed by the Parent/Guardian and the Student.

7.   SOCIAL MEDIA GUIDELINES

Must be read and signed by the Parent/Guardian and the Student.

8.   PHOTO/VIDEO RELEASE

Must be read and signed by the Parent/Guardian and the Student.

9.   CONCUSSION CONSENT FORM

Must be read and signed by the Parent/Guardian and the Student.

10.   CARDIAC ARREST CONSENT FORM

Must be read and signed by the Parent/Guardian and the Student.

When you submit this completed registration form, it will be sent directly to the North Haven High School Nurse. The school nurse will review all parts of this completed registration form to make sure everything is completed and signed by the parent/guardian and the student, including the uploaded physical, the medical history, the permission slip, academic requirements, attendance, code of conduct, social media guidelines, photo/video release, concussion consent and cardiac arrest consent. 

A new athletics registration form is required for every school year. In order to be eligible to participate, the completed registration form must be submitted online to the school nurse 48 hours before the before the first day of participation. Forms not submitted within the specified timeframe may result in the prospective participant not being eligible to participate. *Please note, anyone submitting a registration form the same day as a workout/event begins will not become eligible until the following weekday. A final roster of eligible participants will be provided to the Athletic Director and coach the day before the first workout/event. The coach will also be notified of any ineligible player. 

INJURIES:

1.    The COACH and INJURED PLAYER must report all injuries to the NURSE the following day.

2. Insurance forms and accident forms are to be completed in duplicate. The school nurse has the forms.

3.    WRITTEN RELEASE: If the athlete is seen by a health care provider, a written release is required before participation may be resumed.

4.   THE SCHOOL NURSE SHOULD BE INFORMED OF ANY CHANGE IN MEDICAL STATUS OF A POTENTIAL ATHLETE THROUGHOUT THE SCHOOL YEAR.
