Participation in athletics is voluntary and is not a required part of the regular physical education program. Please complete the following form to help the Athletics department plan for the upcoming school year's Athletics program and to assist the Athletic Trainer and Health Office in conducting an efficient clearance process.
In response to this form, you may receive notice from NEHS that your child needs an updated physician's physical which you should ascertain well before the start of the season. Along with the pre-participation exam, our High School athletes are required to be ImPACT tested prior to participation in athletics at NEHS. ImPACT testing is a computer based test that helps determine when an athlete is ready to return to play from a concussion. All of our athletes are baseline tested before their freshman and junior years. Additional information on ImPACT testing can be found on Mr. Sawicki's webpage. The official sports registrations will open on FamilyID 30 days prior to the beginning of the season. If you do not register your child on the official sports registration form, they will not be permitted to play.
Please choose all the sports you believe your child may be interested in. Final team assignments are at the discretion of the coach.

Please be aware that if your child is injured at school, the school insurance is secondary insurance only. You must first put the claim through your personal health insurance. However, you only have 90 days to submit a claim to the school insurance for reimbursement of your co-payments. Please contact the Business Office at (609) 758-6800 ext. 4210 to get the claim on file, and then you can submit your bills at a later date.
