Registration for Fall Sports is open as of Monday June 1st. Fall sports are scheduled to begin in August with Football starting August 21st and all other fall sports beginning August 24th.
Please note there is an inherent risk in participation in any athletics program.
All students must have a current physical examination recorded with the Health Office prior to the first practice.
Physical exams are valid for 13 months. Sports Physical Exams must be performed by a duly licensed Physician, Physician's Assistant, or Nurse Practitioner.
A sports physical exam must be on file in the nurse's office before any student will be allowed to participate in any sport activity (including tryouts). Students may not participate until cleared by the school nurse.
Sports Physicals will only be accepted under the following conditions:
All sports physical examinations and medical documents must be hand delivered, faxed, emailed or mailed directly to the school nurse. DO NOT give physicals, sports physicals, concussion forms, or any other medical documents to the coach.
The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department & coaches.
