Sports offered this season are as follows:
Boys/Girls Spring Track
Before any student can participate in tryouts, the following requirements must be met:
• PHYSICAL ELIGIBILITY: All prospective athletes must have a current sports physical on file in the Nurse's Office. Physical examinations are good for one calendar year (365 days). If the physical examination on file has been completed more than 90 days prior to the first practice session for the sport, a health history update form (90 day update form) must be completed by the parent/guardian. Please check with the coach or the nurse to determine if the student-athlete has a valid physical for the season. The new physical evaluation and the health history update (90 day update) forms are available on the district website: www.htps.us or under "Links" on the right side of this screen.
NEW PHYSICAL: If the physical on file is not current, a new Pre-Participation Physical Evaluation (PPE) must be completed. The new PPE includes a history form to be completed by the parent/guardian and also a physical examination form to be completed, signed, dated and stamped by the examining physician. Both parts of the PPE must be returned to the school nurse with a stamped, self-addressed envelope. The physical form must be submitted by the due date provided by the Nurse's Office since time is needed for the school doctor to review the physicals before final medical clearance is given. Late submission of sports physicals may be subject to a minimum hold on eligibility for two school days to allow for clearance of sports physicals submitted after the deadline, which may include practice and tryout days. No participation of any kind is permitted without medical clearance from the school doctor and the Nurse's Office. Please make a copy of the completed physical form for your records before submitting it.
If a HEALTH HISTORY UPDATE (90 Day Update) is needed, it must be completed by the parent/guardian and returned to the Nurse's Office. If a Health History Update form previously submitted is more than 90 days old, a new Health History Update form needs to be submitted. All Health History update forms should be brought to the Nurse's Office as soon as possible before the start of the season. No participation of any kind is permitted without medical clearance from the Nurse's Office.
• The HHS Student Random Drug and Alcohol Testing Program Consent-to-Test Form must have been completed and submitted to the Nurse's Office in order to participate in any sport or school activity. The form only needs to be submitted once during the student's high school career. Students can check with the Nurse's Office to see that their form is on file. • PLEASE NOTE: This registration form must be completed by a parent/guardian. The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning the student's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department and coaches.
