HAC Modified, JV, Varsity Spring Sports 2021 Participation Procedures
In order for a student athlete to be cleared to participate in any HAC interscholastic athletic program, the following items must be completed and in the Allendale Columbia Health Office prior to the first day of the sport season:
- A current Health Appraisal form dated no earlier than September 2019. A non-parent physician/ nurse practitioner/ physician's assistant must complete the Health Appraisal form, and the form must be on file in the Health Office.
- Prior to the start of the volleyball season, both the parent and student athlete must complete the Athletic Recertification Form together. This form must be completed, signed, and dated no earlier than one month prior to the start of the sport season (Monday, April 19th).
- Student-athletes taking medication: Since teams have practices and/or competitions after school and on weekends, all athletes must have their Primary Care Provider complete a "Permission to Administer Medication" form for each prescription and/or over-the-counter medication that he/she take except for those medications taken first thing in the morning or at bedtime. Each medication requires a separate medication order with the student's Primary Care Provider's signature.
- Student-athletes with diabetes, a history of a serious allergic reaction or anaphylaxis, or a seizure disorder, must have an "Emergency Care Plan" (ECP) completed by their Primary Care Provider in order to be cleared for practice. If the ECP is completed a "Permission to Administer Medication" form is not needed for medication referenced on the ECP.
These requirements are for all students who participate in interscholastic athletics in New York State in public and independent schools.
