Gloucester, MA High School and O'Maley Middle School Athletics Registration:
PLEASE NOTE: This registration must be completed by a parent/guardian and a student-athlete together. Registration typically takes less than 15 minutes to complete; you must complete it at one "sitting" so ensure that both parent/guardian and student-athlete are present.
This registration program is exclusively for students in the Gloucester Public Schools system. The registrant must be a student at Gloucester High School or O'Maley Middle School to register. The only exceptions are for students in school districts with whom Gloucester Public Schools hosts cooperative athletic programs. After submitting your registration, please be sure to bring your Physical Exam and User Fee (unless paying online) to the Coach, Athletics Administrative Assistant or Athletic Director. The following requirements must be met prior to participation in practice or try-outs:
• Athletics User Fees must be paid prior to practice/try-outs the first day of the season. User Fees may be refunded when necessary. Student athletes will only receive team equipment and/or uniforms after user fees are paid. Outstanding user fees from previous seasons, beginning with Winter 2016-17 fees, must be paid prior to practicing with or trying out for a team.
• All student-athletes must turn in a copy of their most recent Physical Exam prior to the first day of practice. Physicals are valid for participation for 13 months from the date of the exam. Only student-athletes with valid physical exams will be permitted to participate in practices or competition. (Note: arrangements can be made for the Gloucester High School nurse to conduct a sports physical until a student-athlete is able to see his/her physician for an annual exam).
• Parents/guardians are responsible for filling out this form completely and ensuring that all medical information concerning your child's health, medical concerns, injuries, medications, and allergies are reported, via this form, to the Coach and Athletic Department.
• Parents/guardians and student-athletes together must take the necessary online concussion course, read the necessary policies, and discuss and sign the necessary agreements at the end of the registration.
