Dear Parents/Guardians and Students:
The following list identifies the necessary information and forms to clear your son/daughter to participate in Vancouver middle school athletics and activities.
Use this checklist to make sure you have read the required information and completed all necessary forms. Students will not be allowed to practice or participate in competition until the Athletics registration has been fully completed and all forms submitted.
If you decide not to participate after submitting a registration, please let us know, or you might end up with a sports fine.
¨ WIAA Physical Examination form, must be signed and dated by a physician. Physical exams are valid for two years. Please be sure you are using the WIAA Sports Physical form, other forms will not be accepted. Form has to be uploaded in the registration or brought in to the Business office.
¨ Authorization for Administration of Medication in Vancouver School District (only for students needing medication or inhalers). This form needs to be signed and dated by a physician and a parent. The form is only valid for the current school year. Form has to be uploaded in the registration or brought in to the Business office.

Please remember to press "submit" after completing your registration forms in FamilyID for final clearance. Students will not be allowed to practice until all required forms have been completed, submitted and reviewed. This clearing process takes at least 24 - 48hrs.
Fees/Payments: You can pay online or through the Business office. We'll start taking payments at the Business Office on 8/15
- Associated Student Body Card (ASB is required for all activity participants) is $10.00 for the school year; $3.00 replacement fee if lost or damaged.
- Athletic Fee of $20.00 for each sport.
Business Office accepts cash and Debit/Credit card payments.
Families that qualify for free or reduced fees and wish to receive the discount, will need to complete, sign and turn in the Consent to Share form to the Business office. The discounted payment has to be made at the school Business office.
Refund Policy: All middle school have implemented a "no cut" policy for all activities. Students may obtain an activity refund prior to the first contest of a sport season only due to injury or relocation of residence. Fees can be transferred to another sport if for any reason the student cannot participate in the sport for which the fee was originally paid.
