Fall Sports 2020

* * Please sign your student up using the grade they will be in for the 2020/2021 school year!!! * *  NOT THEIR CURRENT GRADE!!!

The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department, coaches and nursing staff. These forms are required by the state and must be taken seriously. Any student filling out these forms and signing for parents/guardians will be suspended from Athletics.

COVID-19 Questionnaire-

All student-athletes must submit a COVID-19 questionnaire, and it must be maintained by the school district.

Student - athletes who have pre- existing medical conditions and/or are immunocompromised (e.g., diabetes, asthma, auto- immune disorders, etc.) Shall provide medical clearance from a physician before he/she will be permitted to participate in workouts.

Physical dates will be posted soon - all sports will have a physical date but only those participating in Fall sports need to register on FamilyID!!

Program Details


• The student athlete must be in the NJSIAA acceptable age range. The student may not have turned 19 years old prior to September 1, of any year. For freshman competition, the maximum age is 16 prior to September 1, of that school year.

• The student-athlete has 8 consecutive semesters of athletic eligibility as determined by his/her initial date of entry into high school.

• Transfer students – ie. Any student who was not a BHS student for the entire previous academic year – must have a transfer waiver as determined by the NJSIAA regulations. This form is available at your previous school.

Sports Physicals- The NJ Department of Education has stated each student who requires a physical must have the examination completed by the family physician or the school physician. The school's physical form must be signed by doctor and stamped by the doctor's office. All athletic candidates must have a physical examination completed by examining physician within 365 days prior to the start of the athletic season for that designated sport. (Physical Form is in 'Links' on the right) Physical forms must be turned into the Nurse and the Nurse will advise parents when the student has been cleared.
