We are excited to announce that we are now offering the convenience of online registration through our partner company, FamilyID. The FamilyID program will help the athletic department and the school district’s families have a more efficient way of completing the sports physical process. The logon and password you create will be used by you for the duration of your child’s (children's) enrollment at Central Regional.
In order for your child to be cleared physically to participate on an athletic team, the 2-step process detailed below must be completed.
If your child has a "cleared" sports physical evaluation packet on file with the school nurse that does not expire until after the start of the season (August 17th) you will just need to complete Step 2, which replaces our “Old Short Form.” Unless they have a pre-existing meidcal condition and/or are immunocompromised. See below statement for additonal medical clearance.
Step 1 -Pre-participation Physical Evaluation – HISTORY FORM & PHYSICAL EXAM:
All student athletes need to have a valid sports physical, recorded on the required Sports Physical Evaluation Packet (Click Here) and please also print the School Physician Cover Page (Click Here). The Sports Physical Evaluation Packet requires the signature of a health care provider that conducted the evaluation. You must print these pages, bring them with you to your medical provider and submit them to the school after the evaluation. Packets along with the cover page should be handed into the Main Office during summer hours from 8:00 am to 2:00 pm (Monday-Thursday).
*****After consulting with the school attorney on the language of the clarification for students with pre-existing medical conditions and/or are immunocompromised (e.g., diabetes, asthma, auto-immune disorders, etc.). We have been advised that those students will need additional clearance to start workouts this summer or any of the athletic seasons during the 2020-21 school year. Besides the normal documents (action plan etc.) the nurse usually requires, these students will need to get a note from their physician prior to beginning workouts that state that they are medically cleared to participate in athletics.
Physicians who conduct the Sports Physical Evaluation must be a licensed physician, advanced practice nurse or physician assistant; and have completed the Student-Athlete Cardiac Assessment Professional Development Module.
All Sports Physical Evaluation Packets must be reviewed by our medical staff and our school physician per NJ state law for your child to be cleared to participate.The health office will deliver the forms to the school physician for review.
Sports physicals are valid for 365 days and need to be active on the official NJSIAA start date for the respective sport being played.
Nurse Medical Forms for conditions such as asthma, diabetes, severe allergic or anaphylaxis, etc. can be found on the FamilyID site under (Links). These forms must be filled out, signed by your physician and handed back into the school for clearance in order for your child to participate in sports on a yearly basis.

Step 2 - Register Online with FamilyID:
The Central Regional Athletic Department uses the FamilyID athletic registration program. You must register for a team each sports season. Failure to register each season with FamilyID will prevent your child from being cleared to participate. All sports registrations must be completed by a parent or legal guardian. Students are not permitted to register on FamilyID.

Per the State of New Jersey and the Scholastic Student-Athlete Safety Act
With the passage of the Scholastic Student-Athlete Safety Act, New Jersey has set the standard by requiring that thorough evaluation be conducted for the silent cardiac abnormalities that may lead to the sudden death of a student-athlete. The below requirements are mandated by the state of New Jersey without exception to participate in athletics for the 2020-2021 school year.
