The Fall Mountain Athletic Program aims to provide an environment that encourages all people participating in our interscholastic sports to receive a rewarding and competitive experience. This experience aims for standards that help individuals gain knowledge and competency in the fundamentals of life and sport. Characteristics associated with these values and standards are;
- Displaying good sportsmanship; and
- Actions of an appropriate and respectful and safe manner as representatives of the Fall Mountain Regional School District.
- The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department & coaches.
- Parent/guardian and the student-athlete should complete an online concussion course in the last 12 months. The free course can be found here- NFHS Learn Concussion Course
