In order to be eligible to play sports at East Valley High School, you will need to complete this form and pay fees to the main office. When all of the requirements have been met, you will be “eligible" to turn out.
It is highly recommended all athletes have a current physical that has taken place in the last year (13 months). However, due to COVID – 19 access to a medical provider to obtain a current physical may prove to be difficult for some. For the 2020-21 school year, if you are a returning athlete and a physical cannot be scheduled, we will accept an approved physical that has taken place in the last twenty-four (24) consecutive months.
Athletes that have not had a physical in the past 13 months, such as new students or new athletes, would still have to obtain a physical before participation. Also any previously injured athlete who received medical attention and who is returning to play would have to present to the District a written release from that medical authority prior to participation. Or if the coach or athletic trainer have concerns about an athlete’s ability to play, an exam may still be needed.
Medical Emergency/Parent Consent/Insurance Form
Medical emergency information, parent consent and updated insurance information must be completed and signed during each registration period. Insurance coverage must include the minimum provisions listed on the parent consent form.
I understand that my son/daughter cannot participate in after school athletics unless the School Accident Coverage Plan or another plan covers him with a minimum payment for any injury of $25,000.
The parent/guardian must sign on behalf of himself/herself as well as the student-athlete (a minor) each registration period.

Sudden Cardiac Arrest and Concussion Form
Player and parental must read and sign the educational information in Concussion Management and Sudden Cardiac Arrest Awareness.
