Families may cancel, reduce or request a change to Student’s Extended Day Schedule by completing this "Extended Day Schedule Change or Cancellation Form" on the Family ID system by the tenth of the month prior to the month within which the family wishes to cancel, reduce or change Student’s participation in the Extended Day program. For example, if the family wants to cancel Student’s participation in Extended Day on November 15, a completed Extended Day Schedule Change or Cancellation Form must be submitted by October 10. If such notice is not provided, the family will be responsible for continuing to pay the applicable Extended Day Fees.
