CV WARRIOR Registration - Fall Sports 2020-21


Registration for Modified, JV or Varsity Fall Sports - grades 7-12

Welcome to Chenango Valley Athletic Department Fall Sports - 2020-21 registration page. Student Athletes, along with their parent/guardian will need to complete this form to be eligible to participate in their selected sport. The Nurses will then review your athletes information to determine if they have a valid physical on file and if they are medically cleared. 

School Health Examinations in Light of COVID-19 Pandemic

Per NYS Guidance ( December 17, 2020) student athletes are permitted to participate in the 2020-2021 sports seasons if they have previously had a health examination for school attendance in the 2018-2019 or 2019-2020 school years and provide a current health history completed and signed by their parent/guardian within 30 days of the start of the season, unless a current health examination is deemed necessary by the school district’s director of school health services (medical director). 

If you have an updated physical you can upload or submit a copy of a physical exam form to the nurse's office. 


  • Completion of this registration is not clearance/approval  to play.
  • You will receive a clearance/approval  to play e-mail from the school nurse upon review of your registration information.
  • Fall Sports, teams, levels, contests, etc...are all depended on local and state approval and guidance.

Golf/Girls Tennis - has been moved to the Spring Season - starting 4/19. 
Athletes can play both their FALL sport (Golf/Tennis) and a their SPRING sport.

Daily Screening Process


The daily health screenings questionnaire, including a temperature check, must be conducted before coming to school to participate in the Warrior Workouts/Athletics. Students will also sign-in upon arriving, indicating they have completed the daily health screening questionnaire.  Parents/guardians and school staff must be instructed that any person, with symptoms or answered YES to the questions below, must stay home and notify your school nurse and medical provider immediately. 

The Centers for Disease Control and Prevention (CDC) keep an up to date list of symptoms of Coronavirus on its website. This list is not all-inclusive as some individuals may display other symptoms or none at all. As of 7/13/2020, the following are listed as the most common symptoms of COVID-19:
• Fever or chills (100°F or greater);
• Cough;
• Shortness of breath or difficulty breathing;
• Fatigue;
• Muscle or body aches;
• Headache;
• New loss of taste or smell;
• Sore throat;
• Congestion or runny nose;
• Nausea or vomiting; and/or
• Diarrhea;

A screening questionnaire determines whether the individual has:

  • Has your child knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive through a diagnostic test for COVID-19 or who has or had symptoms of COVID-19? 
  • Has your child tested positive through a diagnostic test for COVID-19 in the past 14 days? 
  • Has your child experienced any new or worsening symptoms of COVID-19 listed above?
  • Has your child experienced a temperature of greater than 100.0°F in the past 14 days?
  • Is your child required to quarantine due to travel covered by the NYS COVID-19 Travel Advisory?
  • NYS's COVID-19 Travel Advisory requires everyone who travels outside New York State and its bordering states (Connecticut, Massachusetts, New Jersey, Pennsylvania, and Vermont ) to complete the following upon return to New York:

    • If you were away for less than 24 hours, no quarantine is required, but you must take a COVID diagnostic test 4 days after your return to New York.
    • If you were away for 24 hours or more, complete one of the following quarantine options:
      • Quarantine for 10 days
      • Obtain a COVID diagnostic test within 3 days before returning to NYS. Quarantine for 3 days when you return. On day 4, obtain another COVID diagnostic test. Once you get back negative results from both tests, you may end quarantine early.

If a student or staff member becomes ill while participating he/she will be required to stop the activity, be separated from the group, and sent home as soon as possible. The staff member will contact the parent/guardian and Athletic Director.  The school nurse will follow up in the morning. 

Students and staff who are sent home must seek an evaluation from their health care provider and will be required to provide a COVID-19 test result and a note from their physician with clearance prior to return.
