BEFORE YOU REGISTER:
Please read this letter entirely for guidelines on submitting information on FamilyID and to the Cranford High School Health Office. It will be helpful to have on hand your physician’s information, health insurance information, and emergency contact information.
New Physicals are required every year. A physical is valid for an entire calendar year (365 days). There are two options for physicals, one is to have your child attend the CHS In House Physical, or to go to your private physician. Please see details for each below:
PHYSICAL AT CHS: The date for Cranford High School In House Spring Sports Physicals will be as follows:
Friday, February 21 at 3PM
No make up dates will be provided. If your child cannot attend, he/she must go to a private physician.

Complete Registration and Consents on FamilyID including new video
Print out and complete the following forms:
Health History Form
Athlete with Special Needs This form MUST be completed as it is a state requirement for all students.
These forms can be found on the Cranford High School website under Athletics or on FamilyID.
Pre-Physical - To be completed by your child’s school nurse.
Have your child bring both the above forms to their Health Office. Once received, the nurse will complete a Pre-Physical (blood pressure, pulse, height, weight, vision screening). DEADLINE: The pre-physical and all paperwork must be completed and submitted to the Health Office by Tuesday, February 18th.
FamilyID registration, consent signatures, completion and submission of Health History and Athlete with Special Needs forms, and pre-physical are ALL required PRIOR to attending the In House Physical. If these are not complete, your child will not be seen by the physician and will need to go to his/her private physician for the physical.
2) PHYSICAL BY PRIVATE PHYSICIAN:
FamilyID registration and consent signatures must be complete PRIOR to physical exam
Print out the following forms (4 pages) located on FamilyID or the Cranford High School website. Your physician needs these forms to complete the physical exam.
Health History - Completed by parent
Athlete with Special Needs (Required by the State for ALL students) - Completed by parent
Physical Examination Form - Completed by physician
DEADLINE FOR PRIVATE PHYSICALS IS February 20th.
Completed Private Physicals must be dropped off at Cranford High School Health Office no later than February 18th and will then be sent out to our school physician for final approval.
PHYSICALS HANDED IN AFTER THE DUE DATE WILL RESULT IN YOUR CHILD NOT BEING CLEARED IN TIME FOR THE START OF PRACTICE/TRYOUTS. PLEASE ALLOW TIME FOR REVIEW BY THE NURSING STAFF AND SCHOOL PHYSICIAN.
No private physicals will be reviewed by school physician after April 1st.
HEALTH HISTORY UPDATE QUESTIONNAIRE
If student has a valid physical on file in the Health Office you still must register online with FamilyID and complete the online Health History Update portion prior to each sport season. Each student whose physical examination was completed more than 90 days prior to the first day of official practice/tryout shall provide a health history update questionnaire completed and signed by the student’s parent/guardian.
