Welcome to Cassadaga sports registration.
For your student to be cleared to play, you must have:
1) Completed FamilyID Registration which includes health information, contact information, and any mandatory policies and agreements.
2) A Current Physical: Either upload an image of your current physical to the FamilyID sports registration or bring/send a copy to the CV Health office. Physicals are offered (free of charge) in the health office to any student registered for sports. If you miss your physical date, you will need to get one from your family medical provider. Physicals must be within 12 months of the start date of the sport.
3) A current Impact test on file: Impact testing is a computerized test for cognitive function that the school keeps on file in case of a head injury. The baseline test is good for two years. If you require a baseline test, you will receive an email with a link for you to complete the test. In case of a head injury, the school nurse will provide athletes with an updated injury test after clearance from medical provider.
After your registration is complete, it will be reviewed by the health and the athletic departments and you will receive an email once you are approved.
