Dear Parents of Eighth Graders:
We are all very excited about our trip to Washington in March! This tradition has been in place for many years and has long been a rite-of-passage for our eighth grade students. This year the trip will take place from Wednesday, March 11, 2020 to Friday, March 13, 2020. The trip will include visits to the, Arlington Cemetery, and the Capitol, as well as visits to selected monuments and museums. The trip will depart from the Pondfield Road entrance on Wednesday, March 11th. Students should arrive at 7:00am. to board their pre-assigned bus. The buses will leave promptly at 7:30am.
We will hold a meeting for parents to preview this trip and answer questions you may have on Thursday, March 4th at 7:00pm in the Multi-Purpose Room.
In order to participate in the Washington DC Trip, the following items must be submitted:
1. Form A- Field Trip Permission and Medical Release Form. This information is to be completed by completing the sections below this letter. This must be completed by Friday, February 14th, 2020. All data must be filled in, including emergency contact and special medical needs (allergies, dietary needs, prescriptions, etc.)
2. Payment of $475.00 due by Friday, February 14th, 2020. Payment can be done in two ways. The first is to submit a check to made out to Bronxville UFSD and can be turned into the MS office. The payment can also be paid by credit card. If you choose to pay by credit card there is a 3% processing fee.
3. Form B- Medication Administration Form. If applicable, please upload a copy of a physician signed "Form B" Medication Administration form. The Medication Administration Form, along with all medication must be given to the Health Office by Wednesday, March 4th, 2020. If you are unable to upload, return it to the Health Office, along, with any medications to be dispensed. Our contracted nurse will dispense all medications, as per physician’s instructions. Any and all forms of medication (prescribed or otherwise) must be in the possession of the nurse only. Students may not hold their own medications for this trip. You must fill out (Form B) The Medication Administration Form must be signed AND stamped by your child’s physician and then dated and countersigned by a parent/guardian. Form B is only required if medication is necessary.
We look forward to meeting with all eighth grade parents on Thursday, March 4th, 2020 at 7 p.m. in the Multi-Purpose Room to discuss the details of our trip, including the program, the schedule of events, transportation and any other issues that are raised during our session.
