Athletes Registration: Gloucester High School and O'Maley Middle School Fall Athletics Registration:
PLEASE NOTE: This registration must be completed by a parent/guardian and a student-athlete together. Registration typically takes less than 15 minutes to complete; you must complete it at one "sitting" so ensure that both parent/guardian and student-athlete are present. Note that you must re-register each season and that the majority of the information will pre-populate once you select "participant name."
This registration program is exclusively for students in the Gloucester, MA Public Schools system. After submitting your registration, please be sure to bring your Physical Exam, Proof of completion of online Concussion Course, and User Fee in the form of a check to the Coach. You may also deliver it to the Coach by using the mailboxes in the GHS Main Office, 32 Leslie O. Johnson Road, Gloucester, MA. Ask for assistance from the Main Office staff.
The following requirements must be met prior to participation in practice or try-outs:
• All student-athletes must turn in a copy of their most recent Physical Exam prior to the first day of practice. Physicals are valid for participation for 13 months from the date of the exam. Only student-athletes with valid physical exams will be permitted to participate in practices or competition. Parents/guardians are responsible for filling out this online registration completely and ensuring that all medical information concerning your child's health, medical concerns, injuries, medications, and allergies are reported, via this online registration to the Coach and Athletic Trainer.
• Parents/guardians and student-athletes together must take the necessary online concussion course. Please print one Certificate of Completion of the online Concussion Course and submit it to the Coach. You must read the necessary policies and sign the necessary agreements at the end of the registration regarding concussion protocol.
• Athletics User Fees must be paid prior to practice/try-outs the first day of the season. User Fees may be refunded when necessary. Student athletes will only receive team equipment and/or uniforms after user fees are paid. Outstanding user fees from previous seasons must be paid prior to practicing with or trying out for a team.
Gloucester Schools has a Secondary Insurance Policy for Athletics. Please be aware that, in the event of injury, student-athletes are required to submit all claims to their primary insurance carrier for processing. Any unpaid claims may then be submitted to the Gloucester Schools Secondary Insurance program. Please contact the Athletic Trainer for further assistance.
