Welcome to Northwest Catholic Athletics Registration. We are excited to offer the ease of online registration.

Completion and submission of this form must be received before the student-athlete may participate in any school team-organized practice, scrimmage, or regularly scheduled game contest.

Sudden Cardiac Arrest Awareness Student and Parent Informed Consent 2019-19 Agreement

I have read and understand this document the “Student & Parent Informed Consent Form” and understand the severities associated with sudden cardiac arrest and the need for immediate treatment of any suspected condition.

 I authorize my child to participate in the sport selected above for the academic year indicated above.


Student and Parent Concussion Education Plan and Consent Form

I have read and understand this document the “Student and Parent Concussion Informed Consent Form” and understand the severities associated with concussions and the need for immediate treatment of such injuries.

I authorize my child to participate in Athletics for the current school year.


2019/2020 Athletic Handbook for Students and Parents Consent Form Sign-off

From the Director of Athletics:

After reading through the Athletic Handbook for Students and Parents (located at www.northwestcatholic.org/athletics), all students who wish to participate in sports, as well as the parent or guardian of each student, must sign and return this slip of paper to their homeroom teachers to recognize their comprehension of and willingness to abide by the rules, regulations, and guidelines set forth in this handbook. Student-athletes and parents alike must also be aware that all will be held accountable for their actions according to the policies of Northwest Catholic and the contents of this handbook including all CIAC regulations.

I was provided the opportunity to read the Chemical Health Policy as mandated by the CIAC and Northwest Catholic. I fully understand the danger of using these substances and all of the consequences that are involved. I understand the minimum one hundred eighty (180) day penalty and the fact that use of these substances indicated in the policy can jeopardize my physical health, my eligibility to participate in High School athletics, and my current status as a student at Northwest Catholic High School.


Parent/Guardian/Student Atheltic Consent/Release

I hereby give my permission for my son/daughter to engage in CIAC or Northwest Catholic High School approved interscholastic athletic activities including cheerleading as a representative of his/her school. I also give consent for my son/daughter to accompany the team or group as a member on its out-of-town trips. I understand and acknowledge that organized high school athletics involve the potential for injury which is inherent in all sports. I acknowledge that even with the best coaching, use of the most advanced protective equipment, and strict observance of rules and regulations, injuries are still a possibility. On rare occasions, these injuries can be so severe as to result in total disability, paralysis or even death.

I hereby give my consent, in the event of injury or illness, for emergency medical treatment, hospitalization, or other medical treatment as may be necessary for the welfare of my son/daughter, by a physician, qualified nurse, certified athletic trainer, and/or hospital during all periods of time in which the student is away from his/her legal residence as a member of an interscholastic activity team or group. Further, I hereby waive, on behalf of myself and my son/daughter, any liability of Northwest Catholic High School, the Office of Education, Evangelization and Catechesis, the Archdiocese of Hartford, its agents, or employees, arising out of such medical treatment.
