Welcome to online registration for Sandwich HS & STEM Academy Spring sport programs! Before registering, be sure to read this important information:
Please know, our offerings are subject to change at a moment's notice as COVID-19 related guidance from the Governor's Office, EEA, DESE, MIAA, and local authorities continues to evolve. Members of the Athletics Dept. and our coaching staff will make every effort to keep families informed, specifically via e-mail. For the latest information, we encourage you to follow Sandwich HS & STEM Academy Athletics on Twitter at @swich_athletics.
In anticipation of additional expenses related to COVID-19 mandates and guidelines, especially transportation and the need for PPE, participation fees will not be pro-rated regardless of a shortened season (i.e., 10 or 12 games instead of the traditional 20 in Basketball and Ice Hockey, 6 games instead of the traditional 10 or 11 in Football, etc.).
Any student-athlete who still owes the Department of Athletics a uniform or participation fee from a previous season will not be eligible to tryout, practice, or compete until the uniform is returned, the uniform replacement cost has been paid, or the outstanding participation fee has been received!
Any student-athlete who has been diagnosed with COVID-19 during the 2020-21 school year is required to complete a return to play protocol prior to participating in tryouts or practice. The details of the protocol can be found HERE.
Most sport programs and coaches dedicate the first two or three days to tryouts. Following tryouts, everyone at every level is expected to participate and practice from that date forward until the official end of the season. Direct attendance-related questions to head coaches or the Dept. of Athletics.

Before any student can tryout or participate in a Spring sport program at Sandwich HS or STEM Academy, the following requirements must be met.
1. Student-athletes must have a valid physical in order to participate in any athletics program at Sandwich HS & STEM Academy. Physicals are good for 13 months from the date of the visit. Per MIAA, this requirement has not changed due to COVID-19. Hard copies must be hand-delivered to athletic trainer, Jillian DeCuffa, scanned and emailed to firstname.lastname@example.org, or faxed to Jillian's attention at (508) 888-7392.
Additionally, student-athletes diagnosed with COVID-19 are required to have a written clearance from their primary care physician in order to return to play. See Ms. DeCuffa for additional information.
As the start of the Spring season approaches, Ms. DeCuffa will work with our coaching staff to determine which student-athletes are required to take the IMPACT test. Student-athletes will be notified, asap.
2. A parent/guardian is responsible for filling out this online form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications, and allergies are reported to the Dept. of Athletics and coaching staff.
3. Student-athletes and parents/guardians must read the Sandwich HS & STEM Academy Student Handbooks which can be found at their respective websites.
4. Parent/guardian must have completed the FREE, NFHS online concussion course within the past 12 months. The course can be found HERE.
IMPORTANT REMINDER: Without proof of a valid physical, student-athletes will not be eligible to tryout for a sport at Sandwich HS or STEM Academy. There will be no exceptions made. Don't wait!
Every Sandwich HS and STEM Academy program requires a minimum number of participants, usually 10. Any program that doesn't meet the minimum number of participants won't be held. Those registrants, of course, will be notified and have an opportunity to choose another program or request a refund.
