Welcome to Lincoln Girls Lacrosse program's registration. We have a 3 part registration:
- Lincoln’s FamilyID Season 3: This registration collects your medical and insurance information, emergency contact information, waivers, and code of conduct. It must be completed in order to get “clearance” to play. Fees will not be collected in this registration.
- Obtain or Renew your US Lacrosse Membership: Go to Lincolnlaxgirls.com for links to USL.
- Lincoln Girls Lacrosse Team Registration in Sports Engine: Since we are a club sport, this registration contains club specific waivers and collects the team fee upon checkout. Go to Lincolnlaxgirls.com to begin you team registration and/or apply for financial aid.
This Family ID form must be filled out by the parent/guardian and athlete together. You only need to fill this out once to participate in Season 3.
In order to participate in Season 3, this online registration must be complete and the athlete must have an up to date physical by the first day they plan to participate.
Season 3 Guidelines:
Preventing the spread of Covid-19: Students and athletes must wear masks at all times. Everyone will use hand sanitizer before and after every practice or game. In addition, shared equipment will be wiped down with sanitizer spray or wipes.
Contact tracing: After registering in FamilyID, your athlete will receive an invitation to Healthy Roster, the program that allows our athletic trainer(s) to confidentially communicate with athletes and families about injuries and recovery. This year we are also asking all athletes and coaches to use Healthy Roster to complete the daily COVID screening questions at home before they arrive at the field for practice or competition. The invitation to Healthy Roster is sent to the address you list under "Participant Email" in FamilyID. Daily COVID screenings will be sent to the athlete's email address you list in this FamilyID registration under "Participant Email". Athletes will receive an email each morning from Healthy Roster asking them whether or not they are experiencing any symptoms of COVID or if they have been in contact with someone who has tested positive in the last two weeks. It is imperative athletes fill this out daily since the email they get back clearing them for practice is their ticket into practice or competition that day. Athletes will not be allowed to participate if they do not have their clearance email.
Physical Exam Form:
OSAA rules mandate that all student-athletes have a physical examination every two years and turn in a specific form. If the athlete has not had a physical within the past two years or the physical is expiring, please schedule an exam and have your medical provider complete the required physical exam form (found in the links section on the right hand side of this page). By Oregon law we need a hard copy of the completed physical form. Please submit hard copies of your forms into the athletic office, the main office, mail them, or fax them to the school (503-916-2700).
